This is CD 134 [010] No Name insulator manufactured by King City Glass Works (Fairmount, IN).

Piece shows two lehr kisses along the upper wire ridge: one to the right of the dot embossing and one next to the left mold line. You can see them in the photo -- results in a "wavy" wire ridge. Also has a shallow ~1/2" base flake on the other side. If interested in purchase, I can forward detailed pics.
