It was customary for the Edison Electric Company of Boston and Boston Edison for decades to employ this type of pole line construction including lower mounted distribution transformers. Some older poles like this one still stand in Edison service territory where 4kv primary distribution is prevalent (there's lots of it...). Note the wood fuse cutout to the right of the transformer.
