This one was found on the Georgia Railroad in Almon, Georgia on a side pin bracket below the cross arms on the pole. I found another CD 121 A.T.&.T.Co on another bracket. It looked like the telephone company used some of the Railroad poles to run a phone line from a road thru town to a farm house.
