I got this one last summer at the YPCIC meet in Milford Mass. LVIV, Ukraine, CD 304.5 power piece. Embossing is very light on this one. Lviv manufactures mostly glass suspensions, and other electrical distribution hardware. Their pintypes that I know of are CD 304.5, CD 522.5, CD 540, CD 541, and CD 638.
