This was the way that guy wires were attached to the pole before down guy fixtures and preform grips. Either a 5/8" space bolt or two 5/8" lags driven in on opposite sides, 16 ga. plates to prevent the wire from cutting into the pole, guy strand wrapped around twice and secured with a 2 or 3 bolt clamp. This particular pole had a 4 way or storm guy.
