The question was asked---How did the lineman get up the pole with all of the open wire in the way? As you can see there is enough room to go up the middle. I don't know what type of currents ran over railroad open wire but for telephone circuits, the worst shock you could typically get would be the zap of ringing current if you happened to be touching the wrong wire as a call came in.
