W.E. MFG.CO. CD 126.4 in aqua. Two unusual aspects. First, the pinhole is way off center (see picture). Second, while the crown embossing is worn (pretty typical), the "Patent Applied For" embossing is outstanding; almost in perfect relief.

There appears to be a small no-glass-missing bruise on the inside of the base; and it can be seen in the picture on the right side. Other than that, I see only a small bb ding on the rear skirt and a shallow chip on the base. It's in great condition for one of these pieces.
