To help in assignment of a potential new CD you should document your piece carefully. This documentation should include at least

1) Good photos (straight on) of the front, back and bottom

2) Drawing with dimensions

3) List all embossing and it's location

The above is an example that James Mulvey did. I assembled them on to one page for easy of viewing (he had about two dozen to send along). If you just have a couple having separate pages for each is fine. It's just to print them out on hard copy and USPS them.
